Hypoglossal nerve injury may be caused by an errant anesthetic needle...(and may cause twitching).

In the absence (or presence) of a surgical procedure in the area, it is probable that the patient had an injection injury or an injury from the local anesthetic agents to both the hypoglossal nerve and the lingual nerve...(which may cause twitching).

Though not necessarily to the extent of hypoglossal palsy, this study suggests that neck injury can cause tongue symptoms.
