Well I am persisting with physio for my left shoulder and neck (it's an upper limb clinic they are not interested in any other part of me) and at least the physio can tell that the muscles on my left are more tense than the ones on my right, but ultimatly that is only dealing with symptoms and says nothing about the causes.

I do think in a lot of cases muscular tension precedes the fasciculations.

Last night my whole body was at it, sometimes you get a night like that, but that was exacerbated by financial worries which have fortunately been resolved in the cold light of day.
